Paperless Sports Eligibility Packet
Any students planning to participate in Athletics in the 2018-2019 school
year will need to follow the process for Athletic Clearance found below.
It is a very easy process to follow.
Any physicals dated prior to May 1, 2018 will not be accepted for the 2018-19
school year (We will not accept physicals from a PA or Chiropractor).
The form must be signed by the doctor and also include their stamp. Once
you obtain the completed physical, you will need to upload it to
Once you complete a clearance for one sport, most of the information you
have entered will be retained in the system. To register for an additional
sport, select New Clearance and after you enter the year, school, and
sport most of your information will auto fill.
It is a good idea to request clearance for all sports you might want to
play so you don't have to wait for clearance later in the year.
The physical form can be
downloaded here or a paper copy can be picked up in the front office. The physical must
be completed by a medical doctor (MD or DO). We will not accept physicals
from a Physicians Assistant or Chiropractor. The form must be signed by
the doctor and also include their stamp. Once you obtain the completed
physical you will need to upload to the website.

NCAA Eligibility Center
In order for a student-athlete to be eligilbe to play sports or earn a
college scholarship at a NCAA I or II school, a stundent must register
with the NCAA Eligibility Center. It is recommended to register during
sophomore year and no later than junior year.
NAIA Eligibility Center
In order for a student-athlete to be eligilbe to play at a NAIA school,
a student must register with the NAIA Eligibity Center. It is recommended
to register during sophomore year and no later than junior year.
